portant;margin-top: 0px !important;margin-bottom: 0px !important\>（一）滿足《中華人民共和國政府采購法》第二十二條規定：

1.具有獨立承擔民事責任的能力；投標人合法有效的營業執照掃描件，或事業單位法人證書掃描件，或民辦非企業單位登記證書掃描件；

2.具有良好的商業信譽和健全的財務會計制度：即提供投標人的本項目開標時間前6個月內任何時間的資產負債表、利潤表掃描件各一份；或提供投標人上年度的經審計的財務報告掃描件；或投標人的本項目開標時間前6個月內銀行出具的資信證明掃描件；

3.有依法繳納稅收和社會保障資金的良好記錄：即提供投標人的本項目開標時間前6個月內任何1個月的依法繳納稅收掃描件和社會保障資金掃描件的相關材料各一份（依法免稅或不需要繳納社會保障資金的供應商,提供相應文件掃描件證明其依法免稅或不需要繳納社會保障資金)；

4.具有履行合同所必需的設備和專業技術能力：即提供：《具備履行合同所必需的設備和專業技術能力的書面聲明》；

5.投標人參加政府采購活動前3年內在經營活動中沒有重大違法記錄的書面聲明，加蓋電子簽章，格式見本文件第七章《投標文件相關格式》。

所投產品為第二類醫療器械時提供投標人有效的第二類醫療器械經營備案憑證，所投產品為第三類醫療器械時提供投標人有效的醫療器械經營許可證，國家另有規定的從其規定。
