p (1)供應商未被列入信用中國網(wǎng)站( )記錄失信被執(zhí)行人或重大稅收違法失信主體或政府采購嚴重違法失信行為記錄名單；不處于中國 ( )政府采購嚴重違法失信行為信息記錄中的禁止參加政府采購活動期間。（以資格審查人員于投標（響應）截止時間當天在信用中國網(wǎng)站 及中國 查詢結(jié)果為準，如相關(guān)失信記錄已失效，供應商需提供相關(guān)證明資料）。

p (2)單位負責人為同一人或者存在直接控股、 管理關(guān)系的不同供應商，不得同時參加本采購項目（或采購包） 投標（響應）。為本項目提供整體設(shè)計、 規(guī)范編制或者項目管理、監(jiān)理、檢測等服務的供應商，不得再參與本項目投標（響應）。投標（報價） 函相關(guān)承諾要求內(nèi)容。

p (3)投標人應具有有效的且與所投項目相適應的醫(yī)療器械生產(chǎn)許可或經(jīng)營許可，即：①如投標人為所投產(chǎn)品的制造商：所投產(chǎn)品為第二、三類醫(yī)療器械，提供有效的《醫(yī)療器械生產(chǎn)許可證》復印件（如國家另有規(guī)定，則適用其規(guī)定）。②如投標人為經(jīng)銷商：所投產(chǎn)品為第三類醫(yī)療器械，提供有效的《醫(yī)療器械經(jīng)營許可證》復印件；所投產(chǎn)品為第二類醫(yī)療器械，提供有效的《第二類醫(yī)療器械經(jīng)營備案憑證》復印件或承諾中標后辦理《第二類醫(yī)療器械經(jīng)營備案憑證》的承諾函（如國家另有規(guī)定，則適用其規(guī)定）。
